Through prevention, education, and community partnerships, we help individuals and families live healthier lives while reducing barriers to healthcare throughout South Florida.

What We Do
We provide comprehensive healthcare focused on prevention, early intervention, and long-term wellness.
Our services include primary care, preventive healthcare, chronic disease management, HIV and STI testing and treatment, PrEP, PEP, DoxyPEP, patient education, care coordination, and community outreach.
Beyond medical care, we help eligible patients connect with healthcare resources, patient assistance programs, and affordable care options. We also provide confidential sexual health services, free condom distribution, and risk-reduction education to support healthier individuals and communities.

Why We Do It
Too many individuals delay medical care because of financial hardship, lack of insurance, transportation challenges, language barriers, or limited access to providers. Community Health Providers exists to remove those barriers by providing accessible, high-quality healthcare before illness becomes a crisis.
South Florida continues to experience one of the highest rates of HIV in the nation, making prevention, early detection, education, and access to care more important than ever. By integrating HIV prevention and sexual health services into comprehensive primary and preventive care, we help reduce stigma, increase awareness, improve health outcomes, and strengthen the communities we serve.
